Case Manager - Foster Care Kinship Care (FCKC) - Albury

  • Pay details-Salary packaging: $38.65 to $41.45 (Level 3.1-3.4) + 12% superannuation and salary packaging up to $15,900 per year.

About the Role

To provide effective client-focused and outcome-based case management and support to families and kin within our Foster Care, Kinship Care (FCKC) Program. Case management will include, but not be limited to, client interviews, working with clients to identify and address their needs, home visits, referrals and advocacy, case planning, safety planning, support, and mentoring, and working with external service systems to promote client connection.

Key Responsibilities:

As the FCKC Case Manager you will:

  • Develop and implement case plans for individual clients, families, kin, children, and young people in collaboration with other engaged services and supports to address their short-term and long-term needs, including permanency planning.
  • Work closely with families to advocate and ensure their voice is captured in all case planning, goal setting and considerations.
  • Work respectfully with individuals and families to ensure their participation in setting case plan goals is both achievable and realistic.
  • Collaborate with and advocate for the needs of Aboriginal people and their extended with the development, implementation, and regular review of cultural case plans.
  • Maintain respectful relationships with stakeholders, government departments and the community to provide an integrated approach to service delivery.
  • Keep accurate records, files and reporting requirements in a consistent and timely manner that meets legislative requirements.
  • Support children and young people in providing minimum monthly support that may require some overnight travel to ensure that the child and young person's needs are ongoingly met
  • Participate in rostered after hours/ on-call duties

What you will bring:

  • Diploma of Community Services (or equivalent); or
  • Bachelor's degree in Community Services (or equivalent) preferred.
  • NSW Working with Children's Check and Current Driver's License
  • Key knowledge of NSW Children and Young Persons (Care and Protection) Act 1998 and Regulation 2012. NSW Child Safe Standards for Permanent Care.
  • Demonstrated experience in working with families facing complex situations including child protection issues to develop and implement client-centred permanency case management.
  • Experience in working in culturally safe ways with Aboriginal people, communities and/or organisations relating to foster care and/or out of home care.
  • Soft Skills: High-level of communication skills (verbal and written), including presentation skills; the ability to use appropriate interpersonal styles, work under pressure within set time frames.
