Are you passionate about trauma-informed care and integrated service delivery?
Anglicare is seeking a compassionate and proactive Connect Well Health Linker to support victim-survivors of family, domestic and sexual violence (FDSV) within a crisis accommodation setting in Goulburn and surrounding areas.
Key Responsibilities
- Coordinate access to health care and social support services for clients in crisis accommodation.
- Build strong relationships with local health districts, private providers, schools, and community organisations.
- Support clients to remain engaged with care plans and access brokerage funding.
- Advocate for clients within broader systems to ensure continuity of care.
- Contribute to data collection and reporting aligned with program funding requirements.
- Promote the Connect Well program through outreach and community engagement.
You'll bring:
- A Diploma or higher qualification in Community Services, Public Health, Human Services, Social Work, or related field.
- Minimum 2 years' experience in a family, domestic and/or sexual violence-related role.
- Proven ability to work with Aboriginal people, communities and organisations.
- Strong skills in care coordination, stakeholder engagement, and trauma-informed practice.
- Excellent communication, advocacy, and conflict resolution skills.
